Interested in applying for a loan? Download and complete the following forms and deliver to our Gunnison or Crested Butte locations. For security reasons please do not email any of the information.

Please be sure to include the following documents:

All Borrowers:

• Federal Income Tax Returns with Schedules for previous two years (please sign).
• W-2 Forms for the previous two years.
• Pay stub from last pay period.
• Checking Account statements for last month.
• Savings, Investment, IRA’s and 401 K statements (most recent).
• Homeowner’s Insurance Policy on subject property.

All Borrowers, as applies:

• Signed purchase contract (new purchase)
• Gift letter if source of down payment on new purchase is a gift.

Self-Employed Borrowers:

• Corporate or Partnership Federal Tax Returns for previous two years (please sign).
